Our Personal Care Workers (PCW) assists in the provision of holistic client focused and consumer directed care in accordance with an established care plan. The PCW's provide for the personal needs and comfort of clients in their own home.
You will be responsible for:
Performing personal care activities within the Client Care Plan and assist our clients with activities of their daily living.Perform the assigned tasks as instructed by the Registered Nurse within the Client Care Plan, including, but not limited to personal hygiene, toileting, nutrition, and mobility.Identifying client care needs, implementing and evaluating the needs of the client in their home and in conjunction with the RN.Collaborate with other members of the client's care team and care givers to deliver effective, holistic and client focused care.Communicate with clients and significant others using a variety of communication channels.Actively follow our policies, procedures and mission and use a range of competencies and knowledge that are applicable to the role of PCW.Work with the RN to oversight tasks such as assisting in changing ostomy appliances, reinforcement of non-sterile dressings, assisting with mobility aids, assisting with prescribed range of motion exercises etc.Performing light housekeeping duties that will facilitate the client's self-care at home including vacuuming, dusting, sweeping, and mopping floors, laundry, cleaning bathrooms and dishes.Full responsibility for ensuring client notes are completed daily and to raise any concerns regarding the needs of the client to the RN/Care Manager immediately.Join us and work with our amazing clients and surround yourself with a supportive and caring team based in our Perth office.
QualificationsCert III in Aged Care / Disability
Current Basic Life Support (CPR) and First Aid Certificate must be current and maintained.
Preferred SkillsA roadworthy and registered vehicle, along with a current Drivers' Licence.Federal Police Clearance Certificate. (must be within the last 6 months).A willingness to travel and be flexible to work various shifts in different locations.Excellent communication and interpersonal skills.The ability to demonstrate empathy with elderly people.Knowledge of the Legislation applicable to the role of PCW in the Home Care Environment including working within the guidelines and Code of Conduct for Nurses and to observe the Community Care Common Standards and Charter of Rights and Responsibilities for Home Care.Demonstrated ability to maintain and promote the safety of clients and self in the work environment including knowledge of emergency procedures, Work Health & Safety, and Infection Control.A passion for learning and keeping up with legislative changes to ensure you carry out your job with care, attention and within the legal guidelines.Sound literacy and numeracy skills.Strong communication skills and attention to detail is essential. #J-18808-Ljbffr
