Working within a multidisciplinary team, provide effective clinical and emotional support to residents, recognise changes in residents' conditions and escalate observations and concerns.
Deliver high-quality, person-centered care to residents and families, including consideration for cultural, spiritual and individual needs Provide personal care to residents with complex needs such as palliative care, dementia support, restorative care and pain management to enhance their wellbeing Support nursing staff regarding assessment, care planning processes, documentation and evaluation of care outcomes Monitor the cleanliness, safety, environment and promptly report issues of concern About you: This is the place for you if you have:
Certificate III in Individual Support, or equivalent (Aged Care) Experience in residential aged care/ home care (desired) Experience with dementia care and palliative care support (desired) Demonstrated abilities to support residents in a fast-paced environment with exemplary communication skills in a team and one-on-one interactions with all levels of care partners Are committed to being part of a workplace culture based on the BEST way of life (Believe, Excel, Support and Trust) Essentials for working in Aged Care: A current Working With Children Check card Current and ongoing flu vaccinations Australian working rights Ability to pass a national police clearance and NDIS Clearance Make a positive difference in your community – apply now!
