About the role: At Uniting AgeWell, Personal Care Workers co-ordinate personal care for residents in accordance with care plans which meet their individual needs and preferences. Working within a multidisciplinary team, provide effective clinical and emotional support to residents, recognise changes in residents' conditions and escalate observations and concerns.
Deliver high quality, person centered care to residents and families, including consideration for cultural, spiritual and individual needs Provide personal care to residents with complex needs such as palliative care, dementia support, restorative care and pain management to enhance their wellbeing Support nursing staff regarding assessment, care planning processes, documentation and evaluation of care outcomes Monitor the cleanliness, safety, environment and promptly report issues of concern We are currently looking to add passionate and dedicated Personal Care Workers to our professional care team at our Strath-Haven Community Facility.
Shift Opportunities Strath-Haven operates on a fortnightly roster. We currently have the following part-time shift patterns available:
AM Shifts: 07:00 to 13:30 (Wednesday, Thursday, Friday, and Saturday) 07:00 to 15:00 (Sunday, Monday, and Tuesday) 07:00 to 15:00 (Sunday, Monday, Tuesday, and Wednesday) – Fixed Term 07:00 to 15:00 (Thursday, Friday, Saturday) PM Shifts: 14:45 to 21:00 (Thursday, Friday, and Saturday) 14:45 to 21:00 (Wednesday, Thursday, Friday, and Saturday) 14:45 to 22:00 (Wednesday, Thursday, Friday & Saturday) 14:45 to 22:00 (Thursday and Friday - Week 1, Thursday, Friday, and Saturday - Week 2) Please note that these shifts are subject to availability and may vary based on current requirements. About you: This is the place for you if you have:
Certificate III in Individual Support, or equivalent (Aged Care) - Essential Experience in residential aged care/ home care (desired) Experience with dementia care and palliative care support (desired) Demonstrated abilities to support residents in a fast paced environment with exemplary communication skills in a team and one-on-one interactions with all levels of care partners Are committed to being part of a workplace culture based on the BEST way of life (Believe, Excel, Support and Trust) Essentials for working in Aged Care: A current Working With Children Check card Current and ongoing flu vaccinations Australian working rights Ability to pass a national police clearance OR NDIS Clearance Make a positive difference in your community – apply now!
