THE SCOPE AND PURPOSE OF THE JOB The Peer Mentor acts as a role model to their NDIS participants. The main objective for a Peer Mentor is to ensure their participants are working towards achieving their NDIS goals. These goals can be achieved through assisting participants to services, assisting with and/or supervising participants to engage in the community, as well as empowering and building participants' capacity.
The NDIS is a major national reform with an insurance-based approach. It is a new way of providing support for eligible people with permanent and significant disability, their families and carers, via individualized services based on a client's level of need.
RESPONSIBILITIES AND DUTIES Community and Home Service• Support children and young adults in their home or community.• Assist children and young adults with a disability to be more active and independent at home and in the community.• Drive and/or escort the participant to appointments or activities as set out in the treatment plan.• Adhere to the policies and procedures of Access Your Supports when working with participants.
Communication• Advise staff during or at the end of the shift of any changes in the health and wellbeing of the participant, or of any incidents which require reporting.• Follow emergency procedures, including contacting Access Your Supports office staff immediately, in case of emergencies.• Consistently respond to emails and referrals from the Peer Mentor team in a timely manner.
Team Work• Work as part of a team, cooperating and collaborating with the broader team and the organization's workforce.• Actively seek out opportunities to work together with other teams on common issues.• Show consideration, concern, and respect for others' feelings and ideas.• Participate in team and Peer Mentor meetings as required.
CRITERIA Essential• Ownership of a personal motor vehicle and mobile phone.• Full Victorian Driver's License and Full Comprehensive Motor Vehicle Insurance.• Cleared NDIS Worker Screen Check.• Current Working with Children Check.
Desired• An awareness of working with children and young adults with Autism Spectrum Disorder.• Demonstrated interpersonal skills to liaise with the participant and their nominated contact(s).• Capacity to work 10-20 hours per week.
