Coordination includes home visits to our aged and vulnerable clients to implement a service to monitor their wellbeing. You will be completing assessments and welfare checks, monitoring/reviewing, reporting and evaluating in partnership with all key stake holders.
About you You will have a qualification which includes a personal care component in the community services field, license, police check and first certificate. Having worked with older persons, you have the ability to conduct home visits and assessments and develop emergency plans, travelling across the region. You have strong written and verbal communication skills, the ability to solve problems and use your initiative, flexibility to adapt and think on your feet, a track record of building effective relationships, and a commitment to providing high quality customer service.
