The Patient Navigator works in collaboration with a multidisciplinary team, including Case Management, Quality, Physicians and APPs, and clinical managers, functioning as a liaison between patients and healthcare professionals. The Patient Navigator is involved with goals of care, especially as it relates to chronic conditions, critical illnesses, challenging placements, and helping to provide education, resources, advocacy, and mitigation for complex next levels of care.
Job Specific:Assist and facilitate the transition of care, helping to identify and mitigate barriers to bridge patient and family expectation with treatment goals. (EF)Clarify options to patients on their choices as it pertains to post-acute care. (EF)Solve conflicts and crisis situations with health care professionals and family. (EF)Provide education and resources to staff, patients, and family to help them gain an understanding of complex next levels of care. (EF)Ensure patient self-management support is available when appropriate based on language, cognitive abilities, literacy levels, learning style, cultural norms, patient preference, readiness for change, and resources available. (EF)Serve as a supportive resource for staff, patients, and family, assisting clinicians in explanation of care planning. (EF)Maintain professional growth and development through education of current, evidence-based healthcare practices and trends.Complies with applicable regulatory agency guidelines, and federal, state, and local laws, rules, and regulations. Required:Minimum of 5 years' healthcare experience.
