Key Responsibilities: Deliver personalised support to frail, older clients (65+ or 50+ for Aboriginal and Torres Strait Islander individuals) to enhance theirindependence at home and in the community.Collaborate with clients, families, and caregivers to create tailored care plans that prioritise individual needs and choices.Ensure equitable access to services that are culturally appropriate and free from discrimination.Provide flexible, timely support, helping clients avoid costly aged care transitions.
