About the role
As an Occupational Therapist (OT) with VMCH, you will be providing a range of individual comprehensive assessments, interventions, and therapies to our elderly clients under the CHSP and HCP funding programs. You will partner with our elderly clients to identify their individualised goals, interventions, and therapies with the aim of providing them and their caregivers with support, guidance, and recommendations to promote independence, safety, and positive health outcomes.
You will also:

Recommend a range of OT interventions including, but not limited to, assistive technology, aids and equipment prescription, home functional assessments, home modifications recommendations, and consumer education.
Complete detailed reports, scope of works, documentation, and communicate with consumers and relevant internal and external stakeholders including My Aged Care, Regional Assessment Services, Case Managers, General Practitioners, and primary health service providers.
Conduct internal and external referrals to other members of the multidisciplinary team, and external funding providers including SWEP and Geat2Go.
Deliver OT specific group-based programs, and multidisciplinary group programs including cognition, balance, and wellness and reablement activities.

About you
As an experienced OT, you can provide occupational therapy assessments and service delivery, including assessments and development of comprehensive plans and report writing. You have strong interpersonal skills and love engaging with a variety of stakeholders. You have strong communication skills and can maintain professional relationships.
In addition, you have:

Tertiary Occupational Therapy qualification with current registration with AHPRA.
Knowledge of various funding streams and service options for clients.
Current Victorian driver's license.

Knowledge and experience of Government funded aged care programs including CHSP, HCP, SWEP Geat2GO and My Aged Care are desirable but not required.
