Position Title: Occupational Therapist

Position Overview: Provide essential Occupational Therapy assessments and support to clients aged 65+ to promote independence and well-being.Develop care plans under a wellness and reablement model, with a focus on culturally diverse, elderly clients.Work collaboratively within a team to achieve client-centered goals, providing sustainable self-management strategies and community-based support.Job Description:

In this role, the Occupational Therapist will offer assessment and management services under the Commonwealth Home Support Program (CHSP) and Home Care Packages (HCP), helping clients maintain their independence and quality of life. The role emphasizes restorative care through a wellness approach that prioritizes autonomy and self-management, enabling clients to remain living in the community.

Key Responsibilities: Conduct holistic client assessments to identify and address specific Occupational Therapy needs.Create individualized care plans that include the coordination and prescription of necessary equipment.Document client progress and adjust care plans based on achievement against set goals.Provide education and resources to promote client engagement in self-care and health management.Ensure secure data handling and accurate documentation for client assessments and follow-up actions.Collaborate with allied health professionals, family members, and caregivers to ensure cohesive and effective care.Skills and Experience: Bachelor's degree in Occupational Therapy and current AHPRA registration.6+ months of experience, ideally with CHSP and HCP client groups.Proven ability to support culturally diverse clients and those with chronic health issues.Strong communication, organizational, and documentation skills.Knowledgeable in the Active Service Model and experience in implementing reablement and restorative care approaches.A current police check and a valid Australian driver's license are required.Benefits: Collaborative, supportive environment with a focus on continuous learning and improvement.Opportunities for professional growth and specialized training in aged care and community-based therapy.Access to a network of allied health and aged care professionals to expand multidisciplinary experience.Competitive salary package and work-life balance with flexible scheduling options.
