As an Occupational Therapist at New Horizons, you'll play a vital role in enhancing the independence, functionality, and well-being of clients within the NDIS frameworks. Your expertise will empower individuals to achieve their goals and participate fully in their communities. You'll work closely with clients, their support networks, and a dedicated team of professionals to deliver holistic, person-centered care.
Key Responsibilities: Client-Centered Assessments: Conduct assessments, including home safety, functional, and workplace evaluations to meet NDIS standards. Independently assess complex participants' emotional, psychological, cognitive, developmental, and physical level of function. Use clinical observations and standardized assessments and interpret findings to formulate treatment goals. Tailored Intervention Plans: Develop individualized, evidence-based care plans to foster client independence and engagement. Collaborative Care: Partner with healthcare providers and clients' families to achieve coordinated outcomes and capacity-building goals. Case Management: Attend case conferences, health care meetings, and provide consultation as needed. Accurate Documentation: Ensure timely and precise records of assessments, interventions, and outcomes to maintain transparency and quality. Continuous Improvement: Engage in clinical supervision and quality improvement activities, contributing to team performance and innovation. The Ideal Candidate
We are looking for Occupational Therapists who are passionate about person-centered care and comfortable in community or telehealth settings. You should excel in empathy, professionalism, time management, and communication, ready to make a difference every day.
Essential Requirements: Qualifications: Bachelor's or Master's degree in Occupational Therapy. Certification: Current AHPRA registration as an Occupational Therapist. Experience: Demonstrated skills in assessments and interventions, ideally within the NDIS sector. Compliance: Current NDIS Worker Screening Check, Working with Children Check, National Police Check, and a valid driver's license.
