The Role Working predominantly within Home Care Package (HCP) and CHSP funding programs. Conduct comprehensive OT assessments for clients, including those with multiple and complex needs. Deliver targeted Occupational Therapy for clients with complex health conditions or age-related care needs. Provide specific OT services and programs, including home assessments and advice on activities of daily living with recommendations involving home modifications, equipment prescription, care planning and education. Involve clients and carers in developing care plans that are person-centred and reablement-focused. Work collaboratively with the wider allied health team, including nurses, physiotherapists and allied health assistants. Face-to-face clinical work, travel between clients with time allocated for administration and reporting. Ongoing liaison with external agencies and suppliers to arrange services and products required by the clients in accordance with policies and procedures. Contribute to a positive workplace culture through respectful and supportive communication. Requirements Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) – Occupational Therapy Board. Current Driver's Licence. Occupational therapy: 1 year (Preferred).
