3 days) Remuneration aligned with NSW Health Levels Southwest Sydney Location - work in community and from home The Role You will be responsible for managing a caseload of participants aged 65 and over, supporting them in maintaining their independence at home.
You will prescribe home modifications and equipment and develop personalised treatment plans based on assessments and client goals.
Collaboration with other healthcare professionals will be essential to ensure coordinated and comprehensive care.
