Job description:About the role
Our Home Health Registered Nurses are key members of an integrated, multi-disciplinary team, responsible for delivering evidence-based, person-centred nursing care to support customers in their own homes.
The Nursing Team Leader is a dynamic position designed to lead, develop, and support a growing team of registered nurses, ensuring they work to the top of their scope while delivering hospital-in-the-home and community care services.
The role is responsible for fostering a high-performing team capable of meeting and exceeding customer expectations while maintaining safety.
Critical responsibilities include managing the safety and risk of a remote workforce and building a robust, engaged team.
The role also involves maintaining a clinical caseload, ensuring best-practice care is delivered across the network, and serving as an escalation point for all staff.
It is expected that the position holder will divide their time between office-based work and community-based care.

About You
Excellent communication and time management skills with proven ability to multi-task Experience in managing compliance obligations, quality and risk The ability to work in a change environment Demonstrated experience working in an acute, sub-acute and/or community-based environment working with a diverse customer group Demonstrated ability to work autonomously and in collaboration with others to coordinate the delivery of care to customers Demonstrated team leader capability and recent experience working in the acute hospital environment will be highly regarded Demonstrated current practice knowledge and minimum 12 months experience in delivering care to customers receiving IVAB via PICC lines, complex wound care including NPWT, and surgical drain tube management will be highly regarded Working knowledge of various funding streams Current driver's license and registered, insured roadworthy vehicle Unrestricted registration as a Registered nurse with AHPRA
