Registered Nurse - District Nursing Service - Horsham Up to 120 hours available – Permanent part time From $45.12 per hour to $52.83 per hour  About the Role The primary purpose of this position is to provide comprehensive nursing care through direct clinical support, advice and education to clients and their carers within the community on aspects of district nursing care and chronic disease management.
District Nurses also provide support and education to staff and volunteers of other services supporting the client in the community.
The Registered Nurse delivers this role as an active senior member of the District Nursing Team.
Skills & Experience Essential: Registered Nurse with current AHPRA registration High level district nursing and/or community based nursing experience and knowledge of contemporary nursing theory and practice with demonstrated clinical expertise and skills to apply this knowledge.
Proven ability to plan, implement and evaluate evidenced based nursing care and educational activities.
Demonstrated ability to work effectively, both independently and as part of a multi-disciplinary health team and in collaboration with groups and individuals.
Demonstrated ability to communicate effectively with clients, family/carers and team members at all levels within the organisation.
Current Victorian Drivers Licence Desirable: Demonstrated and ongoing commitment to professional development.
Experience with Microsoft computer programs including word and outlook.
