About the role: In this role, your primary responsibility will be to manage a caseload of community-based elderly individuals with Home Care Packages.
You will assess their needs, develop personalised care plans, and manage budgets in accordance with program guidelines, to ensure each customer's unique requirements are met.
You'll also onboard new customers and proactively manage existing ones with a person-centred approach.
Collaboration with our Clinical, Scheduling and Service Delivery teams will be crucial to delivering comprehensive care.
In this full-time permanent role, you will have the flexibility to manage your own schedule each week and work from home when you're not on the road visiting customers.
While most of your caseload will be located in Hobart and the surrounding suburbs, occasional travel to Northern Tasmania will be required.
About you: To be successful in this role, you will have: Previous experience in community-based case management (desirable) Experience with Home Care Packages (desirable) Current driver's licence A reliable vehicle with minimum Third Party insurance Strong communication skills, both verbal and written Demonstrated empathy and compassion Excellent problem-solving abilities and critical thinking skills Good organisational and time management skills Cultural competence and the ability to work with diverse populations Advocacy skills to ensure clients' needs are met Collaboration and teamwork skills to work effectively with interdisciplinary teams About KinCare Our vision is to enable elderly Australians to remain living safely in their homes for as long as they wish by providing them with the care and support they need.
