The aim is to facilitate safe timely care for patients who meet agreed clinical and assessment criteria, enabling efficient patient access and flow, Emergency Department (ED) diversion, and earlier ward bed clearance. There are 2 distinct sub-models: Transit Lounge (discharge bay, transit bay): which improves Patient Flow through day prior Estimated Discharge Date (EDD) planning and improved visibility of daily bed capacity helping Emergency Department (ED) NEAT, post-operative surgical demand and reduce pressures on direct admissions transiting into the organization. Acute Ambulatory Unit (Infusion and Day Procedure Service): The same day infusion service is active across the 6 days (Sunday – Friday), while also increasing the ability to service same day ED infusion needs (Blood Transfusion) and pre discharge infusions requirements (IV Antis/Fluids) helping increase patient flow, NEAT and AV offload. Dedicated Day Procedural Bays allow same day procedures (i.e. BMAT, TOV, and Lumber Punctures) helping reduce the reliance on overnight beds, DPU and Cancer Day Unit (CDU) allowing local areas to focus on their core patient procedures.
