Central Coast Local Health District is seeking a passionate and dedicated Nurse Practitioner (NP) or Transitional Nurse Practitioner (TNP) to join our dynamic Health at Home team. In this role, you'll have the opportunity to function autonomously and collaboratively, delivering safe and high-quality advanced nursing care to our patients.
As a key member of our team, you'll conduct nursing assessments, manage treatments, and provide clinical support to patients across both admitted (Hospital in the Home) and non-admitted services. Your expertise will be instrumental in advancing quality healthcare systems within the Local Health District, with a focus on reducing avoidable hospital presentations and promoting early discharge while maintaining exceptional clinical care for patients in their homes. The core aspects of the role include developing specialist care plans in collaboration with the medical team and fostering a collaborative link with other Community Health services on the Central Coast.

About You You have current registration as a Registered Nurse with AHPRA and current endorsement as a Nurse Practitioner or evidence of working towards endorsement. If not endorsed, the Transitional Nurse Practitioner will be appointed temporarily (up to 3 years) to work towards endorsement as a Nurse Practitioner. You hold a current unrestricted NSW Drivers Licence and the ability to travel across sites and conduct home visits as required. Demonstrated capacity to deliver advanced nursing practice in the field relevant to Hospital in the Home and/or acute care and independently assess, investigate, and implement appropriate treatment or referral for patients within advanced nursing practice guidelines. Demonstrated ability to function autonomously and collaboratively within a multidisciplinary team

Where You'll be Working The role will function in a mobile hybrid model across the Central Coast Local Health District with work undertaken at hubs in Gosford, Wyong and Long Jetty as well as conducting client home visits. The Health at Home team is a large multi-disciplinary team working a Monday to Friday roster.
