Join Ceduna Medical Practice as part of an innovative pilot program that offers you the opportunity to work autonomously and collaboratively in a multidisciplinary team, providing advanced healthcare to underserved and vulnerable populations.
About the Role:As a Nurse Practitioner or Nurse Practitioner Candidate, you will play a key role in delivering advanced clinical care.
This includes assessing, diagnosing, treating, and managing acute and chronic conditions, prescribing medications, ordering diagnostic tests, and referring patients to specialists.
You will be instrumental in improving health outcomes through a strategic, patient-centred approach, with a focus on addressing complex care needs and reducing hospital admissions.
This is an exciting opportunity to work in primary care settings, particularly targeting populations such as those with chronic conditions, frail elderly, individuals with mental health challenges, and others with complex or underserved needs.
The role also includes contributing to quality improvement, engaging in collaborative learning, and using digital health tools to enhance patient care.
Work hours will be Monday to Friday and we offer negotiable flexible work arrangements.
Key Responsibilities: Conduct advanced health assessments and make informed diagnosesPrescribe and manage pharmacological treatmentsOrder and interpret diagnostic investigationsCoordinate patient care, referrals, and community-based servicesSupport the development of innovative care models within the practiceScope of practice to be negotiatedPlease note this role is multi-classed as RN/RM3 or RN/RM4, and classification will be determined by qualifications and experience.
