The Opportunity Reporting to the Group Manager, you'll be stepping into an advanced clinical role where you will work both autonomously and alongside a multidisciplinary team.
Your role will be pivotal in enhancing the quality of care and enriching the journey of residents through expert, person-centred care.
This includes direct referrals to specialists, prescribing medications and supporting the broader goal of reducing hospital admissions through proactive, person-centered care planning.
Ideally, you will be a seasoned Nurse Practitioner with experience in Aged Care but also open to candidates who are newer to Aged Care as the team is well supported with the manager also being a Nurse Practitioner.
