The OpportunityReporting to the Group Manager, you'll be stepping into an advanced clinical role where you will work both autonomously and alongside a multidisciplinary team. Your role will be pivotal in enhancing the quality of care and enriching the journey of residents through expert, person-centred care. This includes direct referrals to specialists, prescribing medications, and supporting the broader goal of reducing hospital admissions through proactive, person-centered care planning.
