Seeking AHPRA registered Nurse Practitioner to join WA's Stronger Medicare award winning team. A rare opportunity to shape your role and integrate into a multidisciplinary practice. Pramana Medical is a multidisciplinary team with an established GP/NP model of care. The practice has 6500 patients, whom are primarily First Nations people or from marginalized backgrounds. The team works holistically with the NP and GP, leveraging the skills of their expansive team to deliver optimal outcomes for the patient. The team includes 4 GPs, an NP, highly experienced surgical and chronic disease nurses, a social worker, psychologist, clinical pharmacist, exercise physiologist, chiropractor, podiatrist, sleep specialist, diabetes educator, and psychiatrist working regularly. The clinic has well-established relationships with tertiary specialists and community practitioners to rapidly access specialist support.
This role would suit an experienced NP or an early career NP due to the extensive amount of clinical support the team is able to provide. The goal of the practice is to screen, diagnose, and treat within the same consult, aiming to minimize barriers to commencing treatment and thus close the gap in health inequality while improving clinical outcomes.
The most common chronic conditions seen in the practice are those relating to the social determinants of health care, including diabetes, CVD, drug and alcohol issues, hepatitis, sexual health, and mental health conditions. Experience or specialization in one or more of these fields or Aboriginal health is not essential; however, it is highly desirable. The practice's philosophy is to leverage the skills of different professionals and work at the top of scope to provide everyone with the highest value. We are therefore happy to evolve the existing NP model to build upon the skillset of the individual.
The salary starts at $150,000 for a 12-month full-time equivalent role, with a sign-on and retention bonus. Additional funding is available for direct clinical supervision, training, professional development, resources, and mentorship based on the experience of the NP.
The practice is located in Gosnells, WA. Immediate start is available. Ideally, this role is full-time; however, there is capacity to job share or work part-time. There is the ability to work from home some of the time, with a core value of the practice being to dedicate time to focus on quality improvement and innovation, and already established Telehealth services to enable flexible working arrangements.
Candidate must hold valid AHPRA registration with Nurse Practitioner endorsements (this position is not open to Practice Nurses or Registered Nurses).
