Within a primary health care framework and in accordance with Agency policy and procedures, the Nurse Practitioner (NP) is responsible, within the defined scope of practice, for high-level clinical leadership in the development, provision, and evaluation of the Hospital in the Home (HiTH) Service within Primary Health North that ensures optimal patient/client/family and community outcomes. The NP is responsible for evaluating and implementing changes to clinical practice, initiating research and quality improvement activities, and will act as a leader, advisor, and mentor for other nurses and clinicians to ensure high-level clinical excellence in the field of chronic, complex care.
The Team: HITH-N is a virtual eleven bed ward that provides acute care to Launceston General Hospital (LGH) inpatients in their permanent or temporary residence or in a community setting. It is an interdisciplinary team including medical, nursing, allied health, pharmacy, and support staff who work collaboratively together using a holistic, client-centred approach to provide seamless high-quality care and optimise patient outcomes.

Acting as a leader, advisor, and mentor and working collaboratively with all members of the team to ensure high-level clinical excellenceProviding high-level clinical leadership in the development, provision, and evaluation of the serviceSupporting optimal patient/client/family and community outcomesEvaluating and implementing changes to clinical practiceInitiating research and quality improvement activitiesDetails of Appointment: Fixed term, casual, shift worker position working as and when required, commencing as soon as possible for a two-year period. *Notwithstanding hours to be negotiated with the successful applicant.

Salary: $139,948 per annum (Pro rata). Our Employer 11.5% superannuation contribution is on top of this amount.
Salary range is in accordance with Nurses and Midwives (Tasmanian State Service) Agreement 2023.You'll have access to salary package a range of benefits including living expenses up to $9,010 + $2,650 in meal entertainment each FBT year + other benefits if eligible.
In addition, the following allowances calculated on the salaried incremental point may be available:
