Within a primary health care framework and in accordance with Agency policy and procedures, the Nurse Practitioner (NP) is responsible, within the defined scope of practice, for high-level clinical leadership in the development, provision, and evaluation of the Hospital in the Home (HiTH) Service within Primary Health North that ensures optimal patient/client/family and community outcomes.
The NP is responsible for evaluating and implementing changes to clinical practice, initiating research and quality improvement activities, and will act as a leader, advisor, and mentor for other nurses and clinicians to ensure high-level clinical excellence in the field of chronic, complex care.

The Team:
HITH-N is a virtual eleven bed ward that provides acute care to Launceston General Hospital (LGH) inpatients in their permanent or temporary residence or in a community setting.
It is an interdisciplinary team including medical, nursing, allied health, pharmacy, and support staff who work collaboratively together using a holistic, client-centred approach to provide seamless high-quality care and optimise patient outcomes.

The Nurse Practitioner (NP) within the HITH-N service is responsible for:
Acting as a leader, advisor, and mentor and working collaboratively with all members of the team to ensure high-level clinical excellence Providing high-level clinical leadership in the development, provision, and evaluation of the service Supporting optimal patient/client/family and community outcomes Evaluating and implementing changes to clinical practice Initiating research and quality improvement activities Details of Appointment:
Fixed term, casual, shift worker position working as and when required, commencing as soon as possible for a two-year period.
*Notwithstanding hours to be negotiated with the successful applicant.

Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full-time, part-time, and casual vacancies.

Salary: $139,948 per annum (Pro rata).
Our Employer 11.5% superannuation contribution is on top of this amount.

Salary range is in accordance with Nurses and Midwives (Tasmanian State Service) Agreement 2023.
You'll have access to salary package a range of benefits including living expenses up to $9,010 + $2,650 in meal entertainment each FBT year + other benefits if eligible.

In addition, the following allowances calculated on the salaried incremental point may be available:
Post Graduate Allowance of up to 7.5% based on qualification type Successful applicants will be required to meet the essential criteria:
Registered with the Nursing and Midwifery Board of Australia and endorsed to practice as a Nurse Practitioner Current Working with Children Registration Registration/licences that are essential requirements of this role must remain current and valid at all times whilst employed in this role and the status of these may be checked at any time during employment.
It is the employee's responsibility to ensure that registration/licences remain current and to advise the Employer if their circumstances change.
This includes notifying the Employer if a registration/licence is revoked, cancelled, or has its conditions altered.
