The Nurse Practitioner Chronic Disease works in collaboration with the Nursing Director Chronic and Complex Care and the wider Central Queensland Hospital and Health Service program team to support the clinical unit in provision of safe quality healthcare through exemplary clinical nursing standards of practice for the specialty of chronic respiratory conditions and other diagnosed chronic conditions.

Nurse Practitioners will have 20% of their rostered hours allocated away from direct clinical duties to support them to work to their full scope of practice. The Sub-Acute Chronic Care Rehabilitation (SACCR) team provides interdisciplinary rehabilitative care to clients with a range of complex chronic diseases. These services also work to address unplanned medical admissions and support early discharge by enhancing the quality and availability of community based proactive collaborative care. This service uses the interdisciplinary client centred model as described in the SACCR service profile, with student placement and clinical education embedded in this core business.
About the role:A Nurse Practitioner (NP) Chronic Disease is a Registered Nurse who:
Functions autonomously and collaboratively in an advanced and expanded clinical role.Uses expanded skills and knowledge in assessment planning and implementation, diagnosis and evaluation of nursing care required.Works closely with the team to establish and manage the Central Queensland (CQ) Respiratory Rapid Access Service which will form part of the hospital avoidance strategies available across CQ.Assesses and manages recipients of healthcare services using advanced nursing knowledge and skills which may include, however, not limited to:Direct referral of recipients of healthcare services to other healthcare.Prescribing medications.Ordering and interpreting results of diagnostic tests.About you:Mandatory qualification/s: Current nursing registration through the Australian Health Practitioner Regulation Agency (AHPRA).
Nurse Practitioner Masters level qualification and NMBA endorsement to practice as a Nurse Practitioner is mandatory.The successful applicant will be required to undergo the CQHHS Nursing and Midwifery and Health Practitioner Credentialing and Defining Scope of Clinical Practice Sub Committee Nurse requirements as a Nurse Practitioner according to the CQHHS policy.What we offer:Salary information: Total remuneration value of up to $177,797 p.a. is comprised of:
