Our Aged Care team are providing co-managed primary care services to residents in Aged Care facilities throughout Australia.Your New RoleThe onsite role provides advanced nursing assessment, diagnosis, care, and intervention to Aged Care Residents within the scope of the Nurse Practitioner. The Nurse Practitioner provides high-level clinical nursing, leadership, management, and governance to patients. Working collaboratively and in partnership with the wider medical team, the Nurse Practitioner assesses, diagnoses, treats and manages patients' health concerns and promotes a healthy lifestyle with a strong focus on preventative health care. As a clinical expert, the Nurse Practitioner provides clinical leadership within their scope of practice.Benefits Great career opportunities in a leading health organisationOpportunity to be a part of a completely new model of medical careValues-based organisation, focusing on our peopleRewarding, friendly work environmentFull time or Part-time positionCompetitive remuneration packageQualificationsBachelors Degree of NursingAHPRA registration as a Registered Nurse with endorsement as a Nurse Practitioner.Essential criteriaAdvanced clinical experience in the assessment and treatment of illness and injury and health promotion relevant to the role and within individual scope of practice.Demonstrated extensive professional and/or leadership experience.Demonstrated clinical problem-solving skills including conceptual and analytical ability.Advanced interpersonal, communication and negotiation skills (written and verbal).Knowledge, understanding and experience in quality improvement, its practical application in meeting patient's needs and quality service delivery.Knowledge, understanding and experience of research findings to support evidence-based practice.Experience in quality improvement, its practical application in meeting patients' needs and its relationship to strategic development.Sound knowledge of relevant legislation and health regulating acts and legislative obligationsComputer literacy.Evidence of appropriate and continuing professional indemnity insurance.Current Driver's Licence and reliable vehicle #J-18808-Ljbffr
