About the role Permanent full-time opportunity to demonstrate accountability at an advanced practice level for managing and coordinating ongoing care for Aboriginal and Torres Strait Islander consumers/patients within TPCH Outpatients Department and for the provision of specialist nursing advice, consultation, leadership and education to inter-professional team members as relevant to the specific context of the role.
About you Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
Coordinate consumer/patient centred care across the consumer/patient journey.Create partnerships, providing a central point of communication, engagement and coordination for all stakeholders involved in the consumers/patients care.Improve consumer/patient outcomes through the delivery of evidence-based care.Facilitate systems improvement - providing a leadership role within the organisation and act as an agent of change.Health Equity It is expected that all Metro North Health staff, including the incumbent of this role as a valuable member of the Metro North workforce, contribute to the health equity agenda and support the defined six actions that specifically meet the needs of Aboriginal and Torres Strait Islander people within the National Safety and Quality Health Service Standards (NSQHS).
