The Nurse Navigator Maternity is a nurse accountable at an advanced practice level for the provision of specialised knowledge across the scope of midwifery practice, to encompass pregnancy and childbirth needs inclusive of the family. The nurse will coordinate a safe and supportive pregnancy journey for complex high-risk women.
The nurse will work alongside the woman and advocate for her around the management of her identified risks. Most of the pregnancy journey is based in the woman's home/community therefore, the nurse will work in these settings to complement her choices.
About the role:The Nurse Navigator Maternity function is a fundamental part of the Midwifery role. It aims to facilitate the woman's journey in what has become an increasingly complex health system. This facilitation will be achieved by reducing fragmentation, mitigating barriers, education, empowering and coordinating care. The role is intended to transcend and access multiple specialities and augment existing models of care across Central Queensland Hospital and Health Service (CQHHS) as requested.
The following are core principles to the Nurse Navigator Maternity role, coordination of patient-centred care; creating partnerships; improving patient outcomes and facilitate systems improvement.
About you:Current registration as a Midwife with the Australian Health Practitioner Regulation Agency (AHPRA) is mandatory.Proven ability to communicate relevant, prompt and accurate information; is approachable and resourceful in working with senior colleagues and peers; uses good judgement in prioritising courses of action and is respectful to co-workers and consumers.Demonstrates advanced clinical evidence-based skill and knowledge in maternity management and/or acute/subacute midwife practice, service coordination and interdisciplinary care management, evidenced by feedback, recency of practice and achieved clinical competencies.Demonstrate critical thinking and reasoning, advanced problem-solving skills and expert clinical judgement.What we offer:Salary information: Total remuneration value of up to $85,346 p.a. is comprised of:
