The RoleAn opportunity has become available for a Nurse Navigator (Hospital in the Home) to join Gold Coast Health (GCH) where the focus of our 12,000-strong team is on providing world-class health care to people living in the Gold Coast and northern New South Wales region of Australia.You will:Facilitate referral pathways across the continuum of care between the primary community setting and care providers.Utilise contemporary information, research evidence, personal knowledge and experience to support decision making.Advocate effectively through the provision of skilled nursing consultation within healthcare team decision making forums.Engage in quality initiative and research projects relevant to the improvement of the patient care journey.About youTo be successful in this position you will require:Bachelor of Nursing or equivalent.A Masters or higher qualification within a relevant area is highly desirable.Registration with the Nursing Midwifery Board of Australia.Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B.Tuberculosis screening, to identify if the tuberculosis vaccination may be required.Declaration of serious discipline history must be disclosed.Criminal History Screening will be required for new employees (GCH will cover cost).Benefits:

Permanent full time.Base salary ranging from $132,562 - $143,924 per annum.About UsThe Hospital in the Home, Geriatric Evaluation and Management in the Home and Interim Care models of care provide hospital level interventions outside the physical space of the Hospital.
