About us:The Central Queensland Hospital and Health Service (CQHHS) Nurse and Midwife Navigator Service provides a centralised Central Queensland-wide model of delivery, with Nurse Navigator positions operating from community care settings, based at Gladstone, Biloela (Banana), Emerald (Central Highlands) and Rockhampton (Capricorn Coast, Mount Morgan, Woorabinda).About the role:Working within the four pillars, this role aims to facilitate a safe and efficient consumer journey, through the reduction of service fragmentation, educating and empowering individuals and coordinating consumer care.
