Fixed Term Temporary Part-time up to six (6) months (with the possibility of extension), 64 hours per fortnight.
Remuneration package of up to $13,724 p.m., comprising base salary range of $5081.10 - $5516.60 p.f. (+ applicable allowances/benefits) (Nurse Grad 7), recreational leave loading (17.5%), and employer contribution to superannuation (12.75%).

About the role
The Nurse Navigator Service meets the needs of the patient and is intended to be flexible and responsive to Townsville Hospital and Health Service (HHS) needs. Navigators work across multiple specialties remaining focused on the patient and their journey. The role scope is not limited by a disease process. Functionally, this means that the Nurse Navigator care of a patient will grow and adapt with the changing needs of the patient irrespective of the traditional medical or nursing specialities that would define care in other contexts.
The Nurse Navigator facilitates person-centred care, integrated care pathways through coordination of models of care and service access for patients with chronic conditions and/or complex healthcare needs. This facilitation is achieved by reducing fragmentation, mitigating barriers, educating, empowering, improving health literacy and supporting coordinated care through the creation of partnerships with care providers including local and primary care and non-government providers.  The Nurse Navigator will demonstrate clinical expertise, utilise case management principles and primary health care service alternatives, where clinically appropriate, with the aim of reducing avoidable hospital admissions and improving patient outcomes through early and proactive interventions.
Role requirements Qualification: Bachelor of Nursing or equivalent certification relevant to the position is mandatory. Appointment to this position requires proof of qualification and registration with the Australian Health Practitioner Regulation Agency (AHPRA) and possession of a current practising certificate. Drivers Licence: This position requires the incumbent to operate a class C motor vehicle and an appropriate license endorsement to operate this vehicle is required.  Proof of this endorsement must be provided prior to the commencement of duty. Highly Desirable
Possession of a relevant postgraduate qualification in an appropriate field. Such experience may be discipline specific or have a service area focus.
