About us:

The Central Queensland Hospital and Health Service (CQHHS) Nurse and Midwife Navigator Service provides a centralised CQ-wide model of delivery, with Nurse Navigator positions operating from community care settings, based at Gladstone, Biloela (Banana), Emerald (Central Highlands) and Rockhampton (Capricorn Coast, Mount Morgan, Woorabinda).

About the role:

The Nurse Navigator – Aboriginal and Torres Strait Islander employs advanced clinical and professional practice capabilities to deliver a positive consumer centred health experience across the lifespan for consumers with complex and chronic health conditions. The four pillars that underpin the Nurse Navigator role are coordination of consumer centred care; creating partnerships; improving consumer outcomes; and facilitating systems improvement.

About you:


Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA) and a current annual practising certificate are mandatory for this position.While not mandatory, a relevant postgraduate qualification, or working towards one, would be well regarded.Under Section 25 of the Anti-Discrimination Act 1991 (Qld), it is a genuine occupational requirement for the incumbent to be Indigenous to the Aboriginal or Torres Strait Islander community.What we offer:

Total remuneration value of up to $7,112 p.m. is comprised of:
