Qualifications & experience Qualified Enrolled/Registered Nurse with a minimum of 3 years experience; Proven ability to provide technical and practical medical procedural training; Proven medical experience; Particularly: - Enteral Feeding - Epilepsy Management - Infection Control - Tracheostomy Care - Catheter and Bowel care; Experience working with children and/or young people with disabilities; Senior first aid and CPR qualification; Strong verbal and written communication skills; A legal right to work in Australia. Current AHPRA Registration Australian drivers licence (minimum P2) Recent Police Clearance First Aid Certificate CPR NDIS Worker Screening Check NDIS Worker Orientation Module Working With Children Check for employees Manual handling training Behavioural support training Up to date with your COVID19 vaccinations Tasks & responsibilities Develop customer care plans and annual budgets for Home Care Package Customers. Conducting skills assessments, providing training, coaching and guidance to our team of Community Care Workers Review and monitor medications, complex clinical care including palliative care, bowel care/suppositories, catheter care, stoma care, oxygen and more. Monitoring and communicating on the customer's wellbeing and reporting on progress of customers. Develop strategies which will address the identified needs of the customer in consultation with their representative/s where appropriate in accordance with their funding guidelines. Effecting management of the care needs of customers and demonstrate effecting utilisation of available resources. Delivery of contemporary nursing care and services to clients, safely and competently. Monitoring and communicating on the customer's wellbeing and reporting on progress of customers. Ensuring that all documentation is completed in a timely and accurate manner. Ensuring that all services are provided in a caring and respectful way in accordance with Diversity Blueprint's policy and procedures, legislative requirements and program specific guidelines (i.e. DVA Community Nursing) Make a difference through providing compassionate care, building trust and taking the lead to support individuals to live in residential home Enhance the lives of our participants by supporting them to live safely and with dignity at home Work as part of a multicultural and diverse team to ensure continuity and consistency of support to participants A reliable, punctual and mature attitude with demonstrated ability to work with limited direct supervision. An understanding of the importance of personal and professional boundaries, privacy and confidentiality, ethical behaviour, policies and procedures.
