The Position
As a Chronic Disease Registered Nurse Care Coordinator, you will play a pivotal role in delivering tailored care, helping patients manage their health from the comfort of their homes. You'll provide expert support in navigating their care, bridging gaps, and promoting self-management strategies that lead to positive health outcomes.
What You'll be Doing
Conduct comprehensive health assessments, providing tailored recommendations for chronic disease management.
Deliver care coordination services to at-risk patients, ensuring smooth transitions between care settings.
Foster collaborative relationships between general practice, community health services, and acute care providers.
Improve patient experiences, quality of life, and reduce preventable hospitalisations through targeted interventions.
Champion inclusive care for diverse communities, including LGBTIQA+, CALD, and Aboriginal and Torres Strait Islander people.
Engage with patients to set health goals, develop care plans, and support self-advocacy.
What You'll Bring
Qualifications:
Bachelor of Applied Science (Nursing), Division 1 Registered Nurse with current AHPRA registration.
Experience:
Advanced clinical skills in managing chronic diseases and complex care coordination.
