)Job Summary As a Care Ambassador, you are responsible for initiating Happier Ageing experiences for Clients and Caregivers, as well as supporting the daily operations of the business.
In every Client care situation, you seek innovative solutions.
You are able to support with delegated care management from the first visit to the end of service.
You will drive results through the following main duties and responsibilities:
Client First Visits Ensures that the First Visit introduction to the client and caregiver represents the Nurse Next Door brand, culture and premium care experienceAssists in reviewing Clients schedule to clarify any questionsReviews Client Care notes in Alayacare with the caregiver, including the care plan and documentationContinuity of Client Care Facilitates Happier Ageing by helping clients stay connected to their hobbies and interestsAudits Client Care plan, replenishing progress notes, updates care plan, flowsheets, MARs, signature logs, etc.Reviews documentation and follows up with any care-related concernsIdentifies changes in level of care and communicates recommendations with Care Designer in a time-sensitive mannerAccurately follows instructions according to nursing delegationsActions client needs with a sense of urgencyReviews Client experience through NPS, achieves goal of 10, to ensure premium care is providedCreates a positive Caregiver experience — Caregivers are set up for success in their role, they have client information and necessary training to provide premium careOversees staff performance - ensures caregiver is the perfect match for every client assignmentDelegates care and provides training and support as neededFosters a spirit of Happier Ageing amongst Caregivers to ensure a premium employee experienceReviews Caregiver experience through NPS, achieves goal of 10, to ensure premium employee experience is achievedSupports with client visits as needed, according to caregiver cancellations and urgent careAdministration Reviews client and staff records to ensure accuracy and completenessDirects the maintenance of client & personnel records to meet employee, organisation and statutory requirements and entitlementsAvailability Flexibility is requiredPart time/Full time/Casual for the right applicantSome on-call evenings and weekends may be requiredBusiness Relationships Reports to the Care Designer (Registered Nurse)Works closely in partnership with the Caregivers, Nurses and Managing DirectorsKey Metrics Consult ConversionDriving RevenueNPS; Client and Employee Score of 10Qualifications Education Requirements AHPRA registration - EN, EEN or RNNursing background is requiredCPR and First Aid CertificationNational Police Check (Aged care specific)Working with Children CheckDisability Workers Exclusion Team Check (to be completed by Nurse Next Door)Car and valid driver's licenseExperience Requirements Exceptional written and verbal communication skillsIntentional organisation, planning, time management and multi-tasking skillsCustomer service and leadership background considered an asset #J-18808-Ljbffr
