We're on the lookout for a passionate Nurse Assessor to join our dynamic team and deliver exceptional, personalised care to individuals throughout the South West.In this rewarding role, you will: Deliver Expert Nursing Assessments: Conduct in-depth initial assessments, develop tailored care plans, and ensure Clients receive the highest standard of care.Collaborate with Healthcare Professionals: Partner with GPs, pharmacists, and specialists to create comprehensive care plans that address all of the client's needs.Onboard New Clients with Compassion: Guide new Clients through the onboarding process, ensuring a smooth transition and personalised support from day one.Manage Medication and Care Plans: Keep Clients' medication records up-to-date and work closely with healthcare teams to provide ongoing care adjustments as necessary.Support Clients and Their Families: Provide not just physical care, but emotional support to Clients and families, ensuring they feel heard, valued, and understood.Drive a Multidisciplinary Care Approach: Work alongside an expert team of healthcare professionals to ensure the best possible outcomes for individuals living with neurological conditions.Maintain Confidentiality & Compliance: Handle client information with the utmost confidentiality and stay compliant with industry regulations, including the Privacy Act and NDIS.What can you expect?This role offers flexibility with 60 hours per fortnight, no weekend work, and a balance of autonomy and teamwork. While your main base will be at our Bunbury Service Centre, you'll also have the opportunity to work at the Vasse Service Centre and travel throughout the South West to support our Clients in their homes and communities.
A bit about you…You're an experienced Enrolled Nurse (EN) or Registered Nurse (RN), with a heart for community care. You have a strong understanding of neurological conditions like Multiple Sclerosis and Motor Neuron Disease, and you're skilled in catheter and wound care. You're a compassionate communicator who can engage with clients and their families, and you thrive in both independent and collaborative work environments. You're ready to bring your expertise to a role where you can truly make an impact.
To succeed in this role, you'll need:Current registration with the Nurses Board of Western Australia as a general nurse.Competence in catheter and wound care.Experience as an EN or RN, ideally in a community healthcare setting.Current First Aid & CPR certificate.Current Driver's Licence and access to a reliable motor vehicle.Working with Children Check.And the benefits?As a not-for-profit organisation, we offer generous salary packaging options—currently up to $18,500 can be packaged to cover living expenses, mortgage payments, or car leasing, meaning you pay less tax on your income!
