About the Role As one of our In-Home CHSP Coordinators, you'll be the primary contact and advocate for our valued clients. You'll build strong relationships, understand their unique needs, and create individualised care plans that promote wellness and reablement. What You'll Bring Experience: Demonstrated experience as an In-Home Coordinator, with a strong understanding of the Commonwealth Home Support Programme (CHSP). Client Focus: A genuine passion for working with older people and a commitment to providing exceptional customer service. Communication Skills: Excellent communication and interpersonal skills to build rapport with clients, families, and colleagues. Care Planning Expertise: Proven ability to create and implement person-centred care plans that focus on wellness and reablement.
