About The Role
Planner/Support Coordinator
Responsibilities of the role include: Leading and driving the development of relationships with new and existing participants and their support networks.To be the NDIS expert supporting a caseload of participants to understand their NDIS plans and how Interchange Illawarra's supports and the supports of other agencies can meet their needs, maximising outcomes and supporting their development of choice and control.Ensuring participant continued satisfaction with Interchange Illawarra services and the supports of other agencies.To support Interchange Illawarra's commitment to person centred support that will put each person at the centre of service design.To be proactive, quick to respond, and focused on achieving outcomes.To be resilient, adaptable, and able to work autonomously.Planning and problem-solving skills.Ability to work within a team and with other organisations.Key Selection Criteria Qualification in Social Science/Welfare or other relevant studies minimum Cert IV.Demonstrated experience managing NDIS Plans and Coordinating Supports experience, planning and delivering supports in the NDIS environment.Ability to work collaboratively with families and other professionals – demonstrate excellent interpersonal skills with the ability to actively listen and build rapport.Demonstrated time management and administrative skills - the ability to multi-task and prioritise work.A demonstrated commitment to innovative person-centred support and quality service delivery.Digital literacy, particularly using Microsoft products.Current driver's licence with registered, insured, and reliable vehicle.Completed NDIS Worker Orientation Module.Smart phone.The successful applicant will be required to undertake a Working with Children Check, police check and NDISWSC.
