About the Role Assist NDIS participants to coordinate and implement a range of support services, in line with their NDIS plan, using a person-centred active support approach.
You will establish strong rapport and relationships with a range of stakeholders including People with Disability and/or mental health issues, their family members, NDIS Local Area Coordinators, NDIS Planners, internal and external service providers. This role is permanent part time (30.4 hours per week) where you will have the flexibility to work from home and from our Unley office when required.
Key Responsibilities Strengthen and enhance our client's ability to coordinate supports and participate in the communityRegularly monitor our client's NDIS plan to ensure services are being delivered within their allocated budgetEnsure mainstream services meet their service obligations to clients (i.e. housing, education, justice, health)Provide the client and the NDIA with reports on outcomes and success indicators, within the agreed reporting frequencySkills & Experience Demonstrated experience in working with people with a disability and mental health issuesExperience in a coordination roleExcellent verbal communication skills and a demonstrated ability to engage with clients and their families to support them as participants of the NDISStrong written and computer skills for the creation of plans, programs, reports, correspondence and other documentsEstablished time management, problem solving and conflict management skillsAbility to think creatively and use initiativeSuccessful candidates will be required to clear probity checks including National Criminal History Record Check, NDIS Workers Screening Check and Working with Children Check.
