About the roleJoin the Open Sky Disability Services' team as a part-time NDIS Support Coordinator based in Maylands, WA.
In this critical role, you will work closely with participants to coordinate and facilitate the delivery of their National Disability Insurance Scheme (NDIS) plans, ensuring they receive the best possible support and achieve their individual goals.
What you'll be doing Collaborate with NDIS participants to develop, monitor and review their individualised support plansCoordinate and facilitate the delivery of NDIS-funded supports and servicesAdvocate on behalf of participants to ensure their needs are met and their rights are protectedBuild and maintain strong relationships with service providers, community organisations and other stakeholdersProvide guidance and support to participants and their families/carersMaintain detailed records and documentation in accordance with NDIS and organisational requirementsContribute to continuous improvement initiatives within the organisationWhat we're looking for Relevant tertiary qualification in a human services or allied health fieldMinimum 2 years' experience in a similar NDIS support coordination or case management roleStrong understanding of the NDIS and disability services sectorExcellent communication, interpersonal and problem-solving skillsAbility to work collaboratively as part of a teamDemonstrated ability to manage competing priorities and work to deadlinesCurrent driver's licence and access to a reliable vehicleWhat we offerAt Open Sky Disability Services, we are committed to supporting our employees and promoting a healthy work-life balance.
