Job Overview: We are seeking a dedicated and compassionate Support Coordinator to assist individuals in accessing services and resources that promote their well-being and independence.
The ideal candidate will work collaboratively with clients, families, and service providers to create personalised support plans that meet individual needs.
Key Responsibilities: Assessment and Planning: Conduct comprehensive assessments of clients' needs, strengths, and preferences.
Develop and implement individualised support plans in collaboration with clients and stakeholders.Resource Coordination: Identify and connect clients with appropriate community resources, services, and support systems, including healthcare, housing, employment, and social services.Advocacy: Act as an advocate for clients, ensuring their rights are upheld and their voices are heard.
Provide guidance and support in navigating complex systems.Monitoring and Review: Regularly monitor and evaluate the effectiveness of support plans, making adjustments as needed to ensure clients' goals are met.Documentation: Maintain accurate and confidential records of client interactions, assessments, and progress notes.
Prepare reports as required by management or funding agencies.Collaboration: Work closely with multidisciplinary teams, including social workers, healthcare providers, and community organizations, to ensure coordinated and holistic support for clients.Education and Training: Provide information and education to clients and their families about available resources, services, and strategies for achieving their goals.Job Types: Part-time, Casual
Pay: $32.19 – $40.00 per hour
