Job Ad: Support Coordinator Role Overview: We2Care seeks a dedicated Support Coordinator to assist NDIS participants in implementing their plans, increasing their capacity, and connecting with their community. You will strengthen informal networks, maximize support value, and maintain key stakeholder relationships.
Responsibilities: Action referrals promptly. Serve as the main contact for service issues, complaints, and plan reviews. Provide participants with information to make informed decisions about services. Assist in engaging services and negotiating service details and costs. Link participants to mainstream or community services, if needed. Liaise with plan managers for correct claim categories and funding. Arrange assessments for funding needs. Strengthen participants' capacity to coordinate and manage supports. Assist participants in understanding service agreement responsibilities. Prepare participants for plan reviews and collect necessary reports. Network to help individuals achieve their personal outcomes. Manage conflicts of interest per NDIA terms. Report on goals and outcomes within agreed timeframes. Enhance We2Care's reputation through continuous improvement and innovation. Develop relationships with participants, families, carers, NDIA, service providers, and community members. Personal Attributes: Pleasant, approachable, and compassionate with a 'can do' attitude. Computer literate with Microsoft Office experience. Technical Skills: Understanding of challenging behavior and positive behavior support plans. Experience in scheduling and accurate allocation. Facilitation of support in personal care, health, community participation, and skill development. Budget management, service agreements, and record maintenance experience. Knowledge of person-centered planning and active support. Commitment to Disability Services Act, WHS, and Equal Opportunity standards. Proven knowledge of NDIS Pathway, plan breakdowns, funding, reviews, processes, referrals, and report writing. Excellent written and verbal communication skills adaptable to participants' needs. Excellent time management and caseload prioritization skills. Mandatory Requirements: First Aid and CPR: A current certificate in First Aid and CPR, or the ability to obtain before you start. Current Driver's Licence. Police Check. Working with Children Check (Blue Card): A valid working with children check or the ability to obtain prior to commencement. NDIS Worker Screening Check or Yellow Card. Right to work within Australia. Qualifications: Minimum Diploma in a relevant field of study. Professional Experience: Relevant experience in the human services area. Experience in working with complex clients and families. Considerable relevant experience in case management within disability services. Experience in conducting needs-based assessments. Experience in multidisciplinary case management across settings/domains. Job Types: Full-time, Part-time, Permanent
