This exciting role The Support Coordinator is responsible for connecting people with a National Disability Insurance Scheme (NDIS) Plan to access a range of supports which align with the support items in their NDIS plan and that suit their individual needs, across one or more providers.
Responsibilities Building knowledge of local services and organisations customers can utilise. Providing crisis support to customers. Determining the budget for each support type and advising any relevant plan manager of the breakdown of funds. Liaising with any plan manager to establish the appropriate claim categories and attribute the correct amount of funds. Supporting the participants in implementing NDIS plans. Assessing and managing risk in line with organisational policies and procedures. Resolving problems or issues that arise. About You To be successful in this role, you will possess the below, or have a willingness to obtain:
A minimum of a Diploma in community services and/or related field. Ability to obtain combined disability worker screening and blue card. Be able to provide National police check certificate. Ability to build high quality relationships with external service providers and participants' formal and informal support. Minimum 2 years' experience in relevant position.
