The Support Coordinator will be responsible for managing and coordinating support services for clients, ensuring their needs are met and their goals are achieved. The Support Coordinator will also collaborate with clients, families and service providers to develop and implement support plans, and will provide guidance and advocacy on behalf of clients.Our ideal candidate would haveExperience in a similar Support Coordination or Case Management role such as Senior Support Worker , DES etc.Knowledge of disability services and the National Disability Insurance Scheme (NDIS)Excellent communication and interpersonal skillsStrong organisational and time management abilitiesAbility to work independently and as part of a teamEmpathy and understanding towards individuals with disabilitiesAbility to manage difficult behaviours and scenarios when requiredGood understanding and knowledge of local allied health servicesCertificate IV in Disability or relevant field (preferred)Working with Vulnerable people card with NDIS worker screeningCompletion of compulsory NDIS worker modulesPlease note, we are a dog friendly office.
