About the role: The NDIS Support Coordinator provides individualised support to people who live with disability (including psycho-social disability) to maximise the usage and positive outcomes of their National Disability Insurance Scheme (NDIS) plan.
Work with participants and their families to identify and progress towards goals, build independence, strengthen formal and informal networks, and link participants with relevant local services and supports.
Use recovery-orientated principles to facilitate participant choice and control, promoting self-determination, and building capacity to manage their plans and supports.
About You: Completed or working towards Cert IV in Mental Health/Disability/Peer Work and/or experience in working in Mental Health/Disability sector.
Demonstrated information management and technology skills including: experience with electronic client/customer management systems, and familiarity with Microsoft Office.
Excellent communication skills, particularly with clients, their family, friends and carer(s) that may be experiencing distress and ability to communicate ideas and information.
