About the opportunity We're currently recruiting for 2 full time NDIS Support Coordinator's to join our team located at our head office in Mulgrave. Both roles will be on a 12 month fixed term contract.
Reporting to our Manager of Support Coordination Operations, you'll be responsible for assisting Participants with complex needs to build personal capacity to make decisions and choices about the implementation of all supports in their NDIS plan, and focus on addressing barriers and reducing complexity in the support environment, whilst also assisting Participants by ensuring they are on track to achieve their goals as articulated in their NDIS plan or Care Plans.
A day in the life as a NDIS Support Coordinator with CareChoice, may see you supporting our Participants with:
Support Coordination to assist in implementing their NDIS plan.Coordinate services in Participants' NDIS plans including the management of complex multi-disciplinary teams.Provide advice and consultation with Participants, their families, and other stakeholders in response to complex and challenging situations.Develop action plans following consultation with Participants, their families, and other stakeholders to resolve barriers arising from complexity.Liaise and collaborate with key stakeholders to establish mutual goals of coordinating service delivery for Participants.About you As a calm, resilient and compassionate NDIS Support Coordinator, you'll have greater than 1 years' experience with providing quality care services, implementing NDIS plans and building rapport with Participants in an outcome driven position in the Disability, Youth Welfare, Child Protection and/or community services sector.
Ideally, you'll have:
Relevant tertiary qualifications in an industry related discipline.Comprehensive understanding of the NDIS and Disability Act.Well-developed interpersonal, negotiation and complaints resolution skills.Strong networking and relationship management skills.Sound understanding of SDA.Qualifications required: NDIS Worker Screening ClearanceWorking with Children Check (Employee type)Valid Victorian Driver's LicenceTo APPLY If this sounds like the ideal role for you and you have the skills and experience we're looking for, we'd love to hear from you.
