We are currently seeking an experienced NDIS Support Coordinator that has experience working alongside people with a psycho-social disability.
Key Responsibilities include:
Provide support to clients based on their NDIS plan, implementing all supports within the clients plan including formal, mainstream and funded supports.Build effective relationships with clients, families and stakeholders while maintaining professional boundaries.Ensure regular contact is maintained and recordedSupporting clients through NDIS plan reviewsAssist clients to understand their funded supports and how to utilise their NDIS plan About the candidate: Minimum 2 years experience as a NDIS Support CoordinatorA comprehensive understanding of mental health issues and the challenges individuals faceBe passionate about pursuing a people focused career which genuinely impacts the lives of others.A thorough understanding of recovery and rehabilitation using a strength based and solutions focused practiceUnderstanding of person-centred supportDemonstrated experience working with individuals who live with a mental illness.Ability to work independently and collaboratively in the community and as a team memberGreat communicators dedicated to providing the best service for every personA broad understanding of mental health systems and legislationA Disability Positive Notice card (yellow card)A Children Check Positive Notice card (blue card)A current first aid certificateA current CPR certificateA current driver's licenseFull Comprehensive Car Insurance This is a Part Time (60hrs a F/N) position located in our Nerang office.
