About the roleWe have an exciting opportunity for a full time NDIS Support Coordinator, to join our team in Darwin. The Support Coordinator will assist people with a National Disability Insurance Scheme (NDIS) plan to make choices of services and supports, coordinate the provision of those services and supports, monitor the delivery of same and provide regular reports to required agencies. Work as part of a local and dispersed team to realise the needs, wants, desires and aspirations of the people we support.
The Support Coordinator will provide this assistance to people based across Somerville housing in the NT.
Core duties may include but are not limited to: Work with the participant and (where relevant) their support network including family / guardian to identify supports / programs / activities / services that will enable the participants' needs, wants, desires and aspirations to be met within the allocated funding of the NDIS plan.Obtain quotes for services and present to the participant for consideration.Completes referrals to relevant services.Strengthen and enhance the participant and their support networks (family / guardian's) abilities to coordinate supports.Build the capacity of the participant to achieve greater independence to self-direct services and supports in the longer term.Ensure mainstream services meet their obligations to the participant (i.e. housing, education, justice, health).Minimum Requirements: Current Northern Territory Drivers License.Current National Criminal History Check.Working with Children Clearance (NT Ochre Card).NDIS Worker Screening.First Aid and CPR Certificate.Vaccinations – COVID-19 with Booster and Influenza.Previous experience or relevant qualification in community services, or a similar role.Understanding of National Disability Insurance Scheme (NDIS).Demonstrate ability to plan, prioritise and execute tasks efficiently.Excellent communication and collaboration skills with internal and external stakeholders.A genuine desire to impact positively on peoples lives.Somerville offers: 5 weeks Annual Leave.Salary packaging benefits to receive up to $15,900 of your salary tax-free.A focus on employee well-being, with regular supervision opportunities.Access to training and learning opportunities.An Employee Assistance Program offering free and confidential counselling and support.Here at Somerville, you will always have a voice, you will always be recognised and rewarded for your efforts.
