COVID vaccination also required.Please email CV and expression of interest to Tim ****** will be in accordance with experience and the SCHADS award, with a view to offering a permanent position, after satisfactory completion of a 3 month probation period.Position Description:Reporting to the Team Leader Support Coordination you will support participants, their family and carers in identifying and connecting with appropriate providers of supports with a view to overcoming barriers, developing skills and achieving the objectives of their NDIS Plan.The Support Coordinator plays an integral role for our clients in interpreting their NDIS plan, assisting them to establish the supports in their plan, and develop their capacity over time to coordinate and manage their own supports and address challenges and obstacles which they may encounter.Your primary role will be to:Actively assist participants in successfully coordinating their support needs and in achieving objectives of their NDIS plan.Provide advice to participants and their carers on the evolving environment of the NDIS and services available.Set up a structured approach to support coordination, including utilising and contributing to a repository of relevant resources.Research and identify providers and other relevant stakeholders and build strong working relationships.Prepare reports, monitor expenditure and complete all associated administrative work.Assist NDIS participants resolve crises when they arise, and take steps to mitigate potential risks.Arrange for any assessments required to determine the nature and type of funding required.Link to mainstream and/or community services.Provide assistance to participants to resolve problems or issues which arise, understand their responsibilities under service agreements, change or end a service agreement.Be the key contact for service issues, complaints, major changes, and plan reviews.Assist NDIS participants to prepare for their NDIS plan reviews by assessing whether they achieved their goals and got value for money through their plan, identify solutions to problems experienced in implementing the plan, and consider new goals.Assist participants to decide what actions to take to achieve goals in relation to exploring housing options and life transition planning.Undertake other roles and duties as directed by the General Manager.The role will require demonstration of the following skills:Ability to relate complex matters in an intelligible way to people from all walks of life and of all abilities.Good written and verbal communication skills.High-level of organisational, time management and planning skills, and the ability to prioritise workload.Strong liaison and interpersonal skills, including active listening, diplomacy, conflict resolution, patience, empathy and ability to work in stressful situations.Initiative and willingness to learn in a fast-paced and evolving environment.Ability to think creatively, be self-motivated, work unsupervised and in a multi-disciplinary team.Experience in working with culturally and linguistically diverse communities.Strong communication skills, ability to provide information in accessible form.Solid report writing skills.Current driver's license, own motor vehicle and willingness to use it when needed.Ability to understand the NDIS price guide and flexibility within budgets, the NDIS legislation and rules including provisions relating to reasonable and necessary supports and the role of the mainstream service system.Acting in this role, it will be expected that you:Maintain confidentiality of customers, their families/carers and the organisation.Effective networking internally and externally to assist individuals achieve their desired personal outcomes.Manage and disclose any perceived or actual conflict of interest.Provide NDIS with reports on specific goals, outcomes, and success indicators within the agreed reporting frequency.Enhance AbilitySeer's practice and reputation through continuous improvement, business development, attracting new clients and referrals, and innovation.
