Ndis Support Coordinator

Centre Labour Force/Territory Disability Service – Alice Springs NT Responsibilities: Engage with individuals with disabilities to understand their needs and goals.
Conduct assessments to determine eligibility and support requirements under the NDIS.
Collaborate with participants and their support networks to develop personalized NDIS plans.
Ensure plans align with NDIS guidelines, funding constraints, and participant preferences.
Liaise with service providers to arrange and coordinate support services according to participants' plans.
Monitor service delivery to ensure it meets NDIS standards and participant expectations.
Maintain accurate and up-to-date records of participant information, plans, and progress.
Prepare reports and documentation required for NDIS compliance and reporting.
Advocate for participants' rights and needs within the NDIS framework.
Address any issues or concerns that may arise during the implementation of NDIS plans.
Work closely with relevant stakeholders, including families, carers, and healthcare professionals.
Foster positive relationships with service providers to enhance the overall support network.
Ensure compliance with NDIS policies, procedures, and regulations.
Implement quality assurance measures to enhance the effectiveness of support services.
Provide information and education to participants and their families about the NDIS process and available services.
Stay informed about changes in NDIS policies and communicate updates to stakeholders.
Address any issues or conflicts that may arise during the implementation of NDIS plans.
Seek solutions to overcome barriers and optimize support for participants.
Participate in community events and initiatives to promote awareness of the NDIS.
Contribute to the development of a supportive and inclusive community for individuals with disabilities.
Summary of Role Requirements: Flexible hours available 1 year of relevant work experience required for this role Working rights required for this role Expected start date for role: 01 November 2024 Expected salary: $55,000 - $70,000 per year #J-18808-Ljbffr
