- Engage with individuals with disabilities to understand their needs and goals.
- Conduct assessments to determine eligibility and support requirements under the NDIS.
- Collaborate with participants and their support networks to develop personalized NDIS plans.
- Ensure plans align with NDIS guidelines, funding constraints, and participant preferences.

- Liaise with service providers to arrange and coordinate support services according to participants' plans.
- Monitor service delivery to ensure it meets NDIS standards and participant expectations.

- Maintain accurate and up-to-date records of participant information, plans, and progress.
- Prepare reports and documentation required for NDIS compliance and reporting

6. Stakeholder Collaboration:

Summary of role requirements: Flexible hours available 1 year of relevant work experience required for this role Working rights required for this role Expected start date for role: 25 November 2024 Expected salary: $55,000 - $70,000 per year
